The Community Center for Preventive Medicine “CCPmedicine” is a non-profit community-based social and health services-providing organization established on September, 2013. We offer health care services through a Family-Centered Health Care approach (FCHC) through empowering individual patients, families and communities to take a leading role in promoting their own health with a continual pursuit of being responsive to the client’s physical, psychosocial and spiritual needs. The FCHC model is a Public Health and Palliative Care approach that can be used in clients of all ages and in all health settings.
