Visiting Nurse’s Outpatient Palliative Clinic

Description

Visiting Nurse’s Outpatient Palliative Clinic is open Monday-Friday. Services are provided by Visiting Nurse physicians and nurse practitioners. Patients served are those facing serious or chronic illnesses and are in need of an additional layer of medical support. An initial meeting with a Visiting Nurse provider will be scheduled for an hour, where goals of care conversations are provided. Important documents are explained and reviewed such as Living Wills and Advanced Directives. Symptom management, including pain, anxiety and social dynamics, are addressed by the palliative care team.
